NHS CPFT Primary Care Mental Health Service

Back to Support

The Primary Care Mental Health Service (PCMHS) provides specialist mental health assessment within GP surgeries so people experiencing mental health challenges can access prompt advice and support, receive help in a community setting and experience a joined-up approach to care.

This video gives you an overview of the service:

The Primary Care Mental Health Service used to be called PRISM. It is accessed via referral from your GP. The Primary Care Mental Health Service support anyone aged 17-64 experiencing mental health challenges based on a person’s needs rather than their diagnosis.

For self-help with mental health challenges, take a look at the Keep Your Head website:

If you need support with your mental health, remember you can always reach out to your GP. If you need urgent help, please call the First Response Service on the NHS 111 helpline and select the mental health option.
