Long COVID Support – Meridian PCN

For some people, coronavirus (COVID-19) can cause symptoms that last weeks or months. Meridian Primary Care Network (PCN) is hosting online and in-person group support sessions so you can share experiences with others or talk to Health and Wellbeing Coaches.

If you are registered with a GP surgery within Meridian PCN (see the map for details) and if you are experiencing Long COVID (for more information and a definition of what that means, take a look here), you can attend group sessions that will provide you an opportunity to talk about your symptoms, learn from others how they are managing and meet regularly with Health and Wellbeing Coaches.

You can self-refer to the Meridian PCN Personalised Care Team by email. After an initial assessment, Health and Wellbeing Coaches will work with you either one-to-one or in group support sessions. If you email, remember to say you are asking about Long COVID support and include your name, contact details and which Meridian PCN surgery you are registered with.
